The Problem

ED patients with complex histories present with 5 to 10+ years of EHR data, multiple prior hospitalizations, surgeries, and diagnoses. ED physicians face 2 to 3 minutes to review and have no time for deep chart review. Manual synthesis of key facts (active problems, medications, recent crises) is time-consuming and error-prone; key information is missed, leading to suboptimal care decisions.
